Treatment Options for Boutonnière Deformity

Boutonnière deformity refers to a hand condition in which a finger is bent in a particular way. In most cases, the finger is bent downwards at the middle joint and backwards at the end joint. This condition is typically seen weeks following an injury of the hand or can be a result of progressive rheumatoid arthritis. There are both surgical and non-surgical options available to treat boutonnière deformity.

When Surgery Is Needed

Surgery may be recommended to patients with advanced forms of boutonnière deformity. In the majority of cases, surgery can help improve function and reduce pain. However, surgery cannot always restore the full look and function of the finger. When boutonnière deformity is left untreated for an extended period of time, it can become much more difficult to treat. Surgery is generally recommended when

  • A sizable bone fragment has shifted in the finger
  • The tendon is severed
  • A deformity develops from rheumatoid arthritis
  • The deformity does not improve with a splint

Non-Surgical Treatment Options

Non-surgical treatment is preferred and can be effective if the deformity is treated early. Finger splits are the most common non-surgical treatment option. A splint is used to securely hold the middle joint of the finger in attempt to straighten it. Finger splints can also help prevent the ends of the tendons from separating as the finger heals. A splint may need to be worn for several weeks before the condition of the finger improves.

Other non-surgical treatment options for boutonnière deformity include stretching exercises. Various exercises of the fingers can be used to gain strength and flexibility in the hand and fingers. For example, one finger stretching exercise involves gently bending the tip of the thumb downwards towards the base of the index finger. Hold the stretch for 30 to 60 seconds, rest, and repeat.

Preventing Boutonnière Deformity

While it’s not always possible to prevent boutonnière deformity, there are things you can do to significantly decrease your risk of developing the condition. It’s critical to wear the proper equipment when playing sports. Use gloves that protect your hands and learn how to properly catch and release a ball to prevent injuring your hand or fingers. If your deformity is caused by arthritis or another situation, talk to your doctor about ways to protect your joints.
